OAZ employees have two dental plan options

OrthoArizona offers a choice of two (2) dental plans from Delta Dental

  • Both plans are PPOs (Preferred Provider Organization)

    • Delta Dental PPO Base Plan

      • PPO Base Plan coverage: Preventive 100%; Basic 70%; Major 30%​

      • Annual Maximum: $1,000 per person

      • Orthodontia (children) coverage: 50% with a $1,000 lifetime maximum 

    • Delta Dental PPO Buy-up Plan

      • PPO Buy-up Plan coverage: Preventive 100%; Basic 90%; Major 60%​

      • Annual Maximum: $1,500 per person

      • Orthodontia (children) coverage: 50% with a $1,500 lifetime maximum 

Dental Plan Overview 

Regular dental checkups can help find early warning signs of certain health problems, which means you can get the care you need to stay healthy. Both the Base and Buy-up plans in the following services:

PREVENTIVE SERVICES 

  • Oral Exams (2 in a benefit year) 

  • Cleanings (2 in a benefit year) 

  • X-Rays (once every 3 years)

 

BASIC SERVICES 

  • Fillings 

  • Simple Extractions 

  • Periodontal maintenance 

  • Emergency Treatment 

 

MAJOR SERVICES 

  • Bridges & Dentures 

  • Single Crowns 

  • Complex Extractions 

  • Periodontal & Endodontic Treatment
