OrthoArizona offers a choice of two (2) vision plans from EyeMed

Both plans provide identical coverage for exams, contact lens fit/follow-up, lenses, lens options, medically necessary contacts and includes 40% off an additional complete pair of prescription sunglasses and 20% off non-covered items including non-prescription sunglasses

Frames: $0 copay; 20% off balance over $130 allowance | Frequency: Once every other plan year

Contacts (conventional): $0 copay; 15% off balance over $130

Contacts (disposable): $0 copay; 100% of balance over $130

Frames: $0 copay; 20% off balance over $200 allowance | Frequency: Once every plan year
